Hi,This could be his partial enlistment information.If it is, you could try to obtain his records from the NPRC.Hopefully his didn't burn in the fire of 1973.When you have all the information you can find, complete the Standard Form 180 within this site.If he has a living sibling, it would be best for them to sign the request.
Other ways to find information is to contact the State Archives and/or County Clerk's office where he enlisted and/or, the State Archives/County Clerk's office that he settled down in after the war.He may have registered his discharge papers that will give you information.
If he belonged to a veterans group, like VFW, or was getting VA benefits, they should have information as well.
If he had a military burial, contact the Sexton of the Cemetery and/or the funeral home that handled his burial.They should have some information as well.
