I think it means Great Altar in RC Parish Church (Kostele) in Podkamien (today Pidkamien) > Rohatyn county > Stanislawow (today Ivano-Frankivska) region. Today Ukraine.

The village had both Greek Catholic and Roman Catholic Churches.
