Filia Ann Merriman (daughter of William Merriman and Rebecca Unknown) was born Abt. 1843 in Cannon Co. Tennesee (Woodbury), and died November 1910 in Union Co. Il.(Anna). She married Monroe Spurlock on November 24, 1856 in Cannon Co. Tennesee, son of Francis Spurlock and Sarah Ashford.

Held at Anna State Hospital in the town of Anna in the county of Union and the State of Illinois, on Thursday the 17th day of November in the year of our Lord One Thousand Nine Hundred and Ten on the body of Ann Spurlock.........Deceased. Present: Dr. Watson Grear

Jury of Inquest: Goe. T. Hileman, W.E. Kimbro, AJ. Halterman, W. Davie Wiley and Geo A. Mattingly.

Attest: John A. Worthington.........Forman.

Be it remembered, that an Inquisition made before the undersigned, Dr. Watson Grear Coroner of said County, at Anna State Hospital in said County, on the 17th day of November A.D. 1910, upon the body of Ann Spurlock by a jury sworn according to law, to inquire how the said body came to its death, the witnesses then and there sworn and testified as follows, to wit: Ardena Upchurch being duly sworn said :

My residence is Anna, Illinois. My occupation is nurse. I think Mrs. Spurlock was received on the 7th day of this month. I last saw her in her usual health on the night she was hurt. I believe it was the 11th. It was about 15 minutes till 12 when I saw her after she was injured. She had a cut over the left eye. It was a deep cut. I was the first one that got to her after she was injured. She was sitting up by the bed on the floor. She seemed to know that she was hurt and said her neck was hurt. She said a big Negro came in her room. She was in a single room-the door was not locked. That was on ward 3. She was generally restless up to about 10 o'clock, but was not noisy. I got her up on the bed and put a sheet across her and put a hot compress on her wound. I did not see any other wounds. We called the doctor - Dr. Richey - and he told us what to do. They took her to the hospital two or three days after she was injured. The wound was more than half an inch long. The untidy patients are on that ward. Ardena Upchurch

Kittie Fitzgerald, being duly sworn said: My residence is in Anna, Illinois. My occupation is night supervisor, Anna State Hospital. Mrs. Spurlock came the 7th of November. She was restless - would get up and walk around the room - but was not noisy. I last saw her in her usual health on the night she was hurt - when I made my rounds at 10 and 11 o'clock. She was sleeping then. I saw her soon after she was hurt. I washed the wound and notified the doctor right away. When I went in she said some Negroes had frightened her. I could not tell what she struck her head on, unless she struck it on the bed as she fell. She was sent to the hospital on the second evening after she was hurt. The wound healed and looked pretty well. I saw her every night after she went to the hospital. She died a little after 11 last night. She had no trouble with any of the patients. Kittie Fitzgerald

Dr. R. M. Richey, being duly sworn said: My residence is Anna, Illinois. My occupation is phyisician. Mrs. Spurlock was admitted according to the records on November 7th, 1910 from Union County. Her age is 75. She was a native of Tennessee. The record says she was an epileptic. She had not had a convulsion since her admission here - so far as the nurses know. She was very feeble, considerably emaciated, she had a constant dribbling of saliva from the mouth. There was a considerable degree of emphysema present. The liver was slightly enlarged and tender. Her arteries were very hard and tartuous. Pulse was slow. After her injury she took septic infection from the wound. I noticed that on the following night. The next morning after that both eyes were completely closed. Then she began to have fever. She died at five minutes after eleven last night. R. M. Richey

In the matter of the Inquisition on the body of Ann Spurlock Deceased, held at Ann State Hospital on the 17th day of November A.D. 1910
We, the undersigned Jurors, sworn to inquire of the death of Ann Spurlock..... on oath do find that she came to her death by infection from a wound produced by a fall in an epileptic convulsion.
