The form that I am having problem with is WD AGO Form 38 which has a printed date of 1 Dec 1944.Here is my father's information:

Now to the problems:
Item 11 says:List all significant diseases, wounds and injuries: and then there is listed the following:
A - Muscular Paralysis, General B - Concussion - July 44 - 105 Sta Hosp - Italy - 3 months
(next to this section are the following columns)
1 - EPTS 2 - and for item A and B is typed NO 2 - AMS 3 - and for item A and B is typed NO 3 - IMS 4 - and for item A and B is typed YES 4 - PD 5 - and for item A and B is typed NO
Can anyone explain what those codes are?I am specifically wondering about the IMS 4 since that says YES to the Paralysis and Concussion!
Can anyone explain to me about the Concussion and Paralysis issue?Does that mean my father was paralyzed at one time...or suffered a concussion and was in a military hospital in Italy for 3 months?I know these may seem like stupid questions, but up until a few days ago, this never showed up in any of his military records and he NEVER told us anything about being injured in the war.I would really like to figure out what happened.
Then everything else on the sheet is ok until I get to:
Item #31 - this item says "Neurological Diagnosis" - and the response is:
11 noted - states he has a burning (over) -
AND then it ends and there is no back of the page that was sent to me.
Item #32 - Psychiatric Diagnosis:
11 a and b noted:no abnormalities noted
I DON'T understand this - IS this related to the Item 11 above where they are talking about my father having paralysis and a concussion?
Item #33 Endocrine System - Normal Item #34 - Slight myopic astimatism Item #35 - Vision - corrected/uncorrected
Items 36-39 are not at issue
Item #40 - "In your opinion will wound, injury or disease result in:Disability:NoUntimely Death:No Condition:11, 37, 34, 35
(Are these numbers related to the numbers of the items on the page or something else???If they are related to something else, is there a code sheet?)
Item #41 - In your opinion was wound, injury, or disease incurred in line of duty?11 - yes34-35 - No
(I am confused by this - is this saying that my father was injured in battle?If so, then why do other records show he was never hurt during the war???If he was hurt in the war, should he not have been issued some type of medal for being injured in combat?He was issued medals but they were the standard ones from what I understand - Good Conduct; WWII Victory; Combat Infantry; and the one for European Operations)
Now to the next document - WD AGO Form 53-55
Organization:HQ CO 3 BN 350 INF
(Can anyone tell me what this means and if so, can you tell me what organization this was?)
Battles and Campaigns:GO 33 WD 45 as amended
(My father was at Rome Arno according to this record...but I don't know what this other item is - can anyone tell me what this is?I would like to try to figure it out if possible! I have not been able to determine his actual unit).
I think that is it.I hope someone can help me to decipher this stuff...as I said, I know some of these questions are really stupid...but I don't know much about the military...and I am trying to figure out what happened to my father.There are some things that happened in our life that are now coming to light nearly 30 years after his death that could possibly be explained by his military record, if we can just understand more of what happened to him.IF what happened is what we now think happened, then that will explain a lot about his behavior!I am so grateful to have a site like this to turn to for help.Thank you so very much for any insight and advice!
