On the image, JAN RAYSKY is on the right hand side of the book, about the 8th name listed.MATHILDE BARSAKOVA is directly below his entry.I believe them to be my great-grandparents.

My grandfather actually listed his mother'smaiden name as BARTEK, but I'm not sure he even knew how to spell it.
