NY City 1926 Birth Cert.- Can't Get???

Melomedow, Mildred, NY City, 1926.

I've tried to order a copy of this relative's birth cert. from Dept of Health.NYC says I need a picture ID of myself, the relative and the relatives written, notarized permission to obtain birth cert.This relative is deceased?

Is there another process to obtain genealogy related birth cert.?This is my first attempt at obtaining a birth record.Would prefer certified copy, but just a copy would be better than nothing.
