WIDOW'S DECLARATION FOR PENSION OR INCREASE OF PENSION
This must be Executed before a Court of Record or some Officer thereof having Custody of the Seal.
State of Ohio, County of Jackson, ss:
On this 12 day of July, A. D. one thousand eight hundred and ninety, personally appeared before me, Clerk of the Com. P. Court, a Court of Record within and for the County and State aforesaid, Massa A. Allen, age 52 years, who being duly sworn according to law, makes the following declaration in order to obtain the Pension provided by Acts of Congress granting pension to widows: That she is the widow of Andrew J. Allen, who enlisted under the name of Andrew J. Allen, at W. Va. on the 12 day of Sept, A.D. 1862., in Co. H. 13th W. Va. Infty in the war of 1861-5, who was discharged in July 1865, and who died in Jackson County, Ohio, on the 12 day of April, A. D., 1885, who bore the rank of private in said service; that she was married under the name of Massie Ann Coleman to said Andrew J. Allen, on the ____day of ____A. D. 1852 by A. Simmons Esq, at Rockingham Co. N.C. (Rockford C.H.), there being no legal barrier to such marriage; that neithershe nor her husband had been previously married; that she is without other means of support than her daily labor; that she has to present date remained his widow; that the following are the names and dates of birth of all his legitimate children yet surviving now under sixteen years of age, viz:
None under 16 years of age
that she has not in any manner engaged in, or aided or abetted, the rebellion in the United States; that no prior application has been filed by her for pension; that she hereby appoints, with full power of substitution and revocation, H. C. Miller of Jackson, Ohio, her attorney, to prosecute the above claim; that her residence is No.____street_____ and her Post Office address is Rempel, Jackson Co., Ohio. Massa A. Allen (X her mark)

Also personally appeared Robert McAlister, residing at Jackson Co. Ohio, and James Allen residing at Jackson Co. Ohio, persons whom I certify to be respectable and entitled to credit, and who, being duly sworn, say that they were present and saw Andrew J. Allen*, the claimant, sign her name (make her mark) to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and their acquaintance with her, that she is the identical person she represents herself to be; and that they have no interest in the prosecution of this claim.
